Experience: Minimum 2 years required (VMS notes also reference minimum 1 year in one section; 2 years noted in multiple fields — use 2 years as primary requirement); first-timers not accepted; travel experience required 2. Cases: Preferred — Neonates, Infants, Pediatrics; skills include Acute Hospital OR, Cardiac, Laparoscopic, Orthopedics, Robotics, Neurology, OB/GYN, Ophthalmology, Plastics, Urology, Trauma, Ortho/Totals, Neuro/Crani, and more 3. Certifications: CBSPD and IAHCSMM/HSPA required; no national certs listed as required in scheduling section 4. License: CA license must be in hand at time of submission; pending state license not accepted; LA fire card within the last 30 days required (not reimbursed) 5. Education: Not specified 6. EMR: Epic experience preferred; Mediteach listed as charting system; charting system experience preferred 7. Shifts/Call: Rotating weekends required — at least every 3rd or 4th weekend, may exceed to meet unit needs; 5x8s (3:00 PM-11:30 PM) or 4x10s (1:00 PM-11:30 PM); on-call not required; facility may call off 1 shift per 2 weeks; floating between all Providence facilities within state required — refusal may result in termination 8. RTO: All RTO must be included at time of submission; approval based on facility needs; max 7 days (Aya and client offer); anything over 7 days requires client approval; must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day); block schedule not accommodated 9.
