- Key Requirements Summary: 1. Experience: 2 years of acute hospital case management experience required 2. Cases: Discharge planning, care coordination, biopsychosocial assessments, 5150/involuntary holds, knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, POLST, AIDA, PASRR; ability to organize and participate in family meetings and development of treatment plans 3. Certifications: MSW required 4. License: CA state license preferred; pending license accepted 5. Education: Not specified 6. EMR: Epic required 7. Shifts/Call: Every other weekend required; must be available for holiday coverage; will be required to cover up to 2 units on weekends with focus on discharge planning; patient ratio 1:35; case load up to 25 patients; orientation is 40 hours 8.
