- Key Requirements Summary: 1. Experience: 2 years required for submission (note: 18 months listed separately — refer to job description for exceptions by unit) 2. Cases: All surgical specialties except transplant; must be comfortable scrubbing CVOR and floating to General OR cases; extensive skills list includes CABG, Da Vinci robotic surgery, TAVR, VAD, VATS, EVAR, TEVAR, and more 3. Certifications: BLS and NBSTSA required; must be AHA or equivalent per unit description 4. License: Must be in hand prior to start date; temporary licenses acceptable once permanent application submitted; SSN and DOB required at time of submittal 5. Education: University must be accredited by ACEN or CCNE at time of graduation 6. EMR: Cerner preferred; no specific charting system required for IU Health positions 7. Shifts/Call: Call required every other weekend; orientation is 40 hours; patient ratios 1:1 days, nights, and weekends; floating 100% required within hospital or to affiliate hospitals 8. RTO: Up to 5 non-holiday days auto-approved; anything more requires facility approval; candidate must work 2 out of 3 major holidays 9. Returning Staff: Any PERM or PRN IU employee must be away 12 months before applying as traveler; consecutive travel policy — 12 months accrued at IU Health requires a consecutive break (effective 2/24/26); out-of-state residents require 90-day break; in-state residents require 180-day break before year mark 10. Travel/Local: Indiana residents considered local; locals accepted for Surgical Services, Allied, and IU Clinics positions at lower in-state rate; locals NOT accepted for MS, MS Tele, PCU, ICU, or ER 11. Background: Must be disclosed prior to submission to seek approval before offer 12. Modules: Not specified 13. Travel Experience: Not required; first-time travelers accepted 14. Scrubs: Required scrub skillset confirmed; color or hospital-provided scrubs not specified 15. Flu Vaccine: Required — Medical/Religious Exemptions Only 16. Special Equipment: 2 Xi Da Vinci Robots Critical: 2 supervisor references required. SSN and DOB required at time of submittal. Candidate must confirm willingness to float to General OR cases.
