- Key Requirements Summary: 1. Experience: Minimum 1 year Home Health experience; 2 years overall as a clinician required 2. Cases: In-home Patient Care, Interdisciplinary Care Coordination, Plan of Care Development and Oversight, Geriatric Population 3. Certifications: BLS required; valid license in hand at time of submission; no pending licenses accepted 4. License: Valid state license required in hand at time of submission 5. Education: Not specified 6. EMR: HomeCare HomeBase (HCHB) and Oasis required; 1 year experience with both preferred; charting must be completed while at patient home 7. Shifts/Call: Must be open to 1 night of on-call during the week; weekends as needed; on-call rotation M-F (8a-5p); 30 points per week equivalent to 40 hours; average 24-30 points per week; 4-6 patients per day 8.
