- Key Requirements Summary: 1. Experience: 2 years of acute HD & PD dialysis experience required (HARD STOP) 2. Cases: Hemodialysis (HD) and Peritoneal Dialysis (PD); high acuity patients including ESRD and Acute Kidney Injury; ICU-level 1:1 care required 3. Certifications: BLS and ACLS required; Basic Dysrhythmia and Specialty Dialysis competency assessment with minimum passing score of 80% within the last year required; CNN (Certified Nephrology Nurse) preferred 4. License: Valid RN license required; pending state license NOT accepted; no license hits — immediate rejection if submitted with a license hit 5. Education: Not specified 6. EMR: Epic 7. Shifts/Call: 3x12 Days; on-call required 1-2 days per week; hours must be flexible as acute program may require more than 12 hours; at least every other Saturday required; no block scheduling 8. RTO: No more than 5 days per 13-week contract; NO holiday RTO accepted at all; only one pre-approved time off 9. Returning Staff: Minimum 1-year separation from INTEGRIS as an employee required; 1 submittal allowed per traveler across ALL Integris campuses; no travel pairs 10. Travel/Local: 75-mile radius rule; locals NOT accepted; candidates within 75 miles are ineligible; address on utility bill must be greater than 75 miles from facility; floating required to other areas and sister facilities up to 75 miles 11. Background: No license hits; immediate rejection if submitted with a license hit; any gaps of 3 months or more in work history require explanation 12. Modules: Average 2-8 hours; non-clinical module hours are billable; clinical module hours are non-billable; completed during orientation; vary by specialty; recorded via facility report 13. Travel Experience: First-time travelers accepted 14. Scrubs: Not specified 15. Flu Vaccine: Required — Medical/Religious Exemptions Only; COVID-19 Vaccine also required — Medical/Religious Exemptions Only 16. Special Equipment: Baxter AK 98, portable RO, central RO Critical: Driver's License AND utility bill (in traveler's name, matching address, within last 3 months, address must be greater than 75 miles from facility) MUST be included with submission. Two supervisory references required. Submissions missing DL, containing holiday RTO, or exceeding 5 days RTO will be escalated.
