Experience: 5+ years of RN Case Management experience required per manager; must have Inpatient Acute Case Management (hospital experience); VMS notes state 2 years minimum unless otherwise stated 2. Cases: Inpatient Acute Case Management required; experience with Care Coordination, Discharge Planning, and Utilization Management 3. Certifications: BLS required; must be AHA or American Red Cross; copies with valid expiration dates required at submission 4. License: CA RN License required; Nursys upload within last 2 weeks with valid CA State License 5. Education: Not specified 6. EMR: Epic (Charting System Name listed) 7. Shifts/Call: 5/8s; Every Other Weekend; Weekend Coverage required; Day Ratios 15 patients, Weekend Ratios 25 patients 8.
