Job Description

Epic Travel Staffing is hiring a Travel – LCSW – Social Worker/Discharge Planner

Shift: Days, 8a-4:30p, 8×5, 40hrs per week

Length: 13 weeks

No on call requirement; weekends included in rotation (every 5th or 6th weekend must work one shift); holiday coverage as needed

Requirements:

  • DC License
  • 2 years of experience discharge planning to include: admission criteria review, benefits eligibility, care coordination, conducting needs assessments and order DME; experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, Acute hospital experience
  • Graduate of a CSWE (Council on Social Work Education) accredited program
  • Completion of at least 1 travel assignment
  • Regulatory Skills required: CMS, HIPPA, JCC; 
  • BLS
  • Prior charge experience preferred
  • Workers’ compensation preferred

  • Charting: Cerner
  • No Float
  • Local within 60 miles of facility
  • COVID-19 Vaccine (Facility Guideline): Not Required
  • Flu Vaccine (Facility Guideline): Required – No Exemptions
Social Worker/Discharge Planner
Shifts: 800-1630 (8A-430P) 5 x 8 Days

Local within 60 miles of facility

Requirements:
*Will accept RN Discharge Planner or Licensed Social Worker – Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already*
BLS

Charting: Cerner
Scrubs: Uniform is business casual, no scrubs – professional appearance

This facility requires that all employees have the Flu vaccine
