- Key Requirements Summary: 1. Experience: 2+ years of Home Health experience required 2. Cases: Admission assessment, Case Management, Discharge, Recertification, Transfer, Standard Visit; Adult Home Health and Home Infusion settings 3. Certifications: BLS required 4. License: Active Virginia State or Compact RN license required; pending license accepted only if candidate holds single-state license from non-compact state 5. Education: Not specified 6. EMR: Epic experience required; OASIS assessment experience required 7. Shifts/Call: M-F 8a-5p; No weekend requirement; Facility can call off 1 shift per 2-weeks 8. RTO: ALL requests must be included with submission; requests accommodated only if approved by facility at booking; Must work 1 holiday per 13-week assignment (2 holidays for winter: Thanksgiving, Christmas/Christmas Eve, NYE/New Years) 9. Returning Staff: Previous employees must be gone minimum one year before returning; After 365 days worked, cannot return despite separation period (effective July 1, 2025) 10. Travel/Local: Locals under 100 miles limited to one assignment, no extensions; 0-49.9 miles: $5 less than posted rate; 50-99.9 miles: same rate 11. Background: Not specified 12.
