- Key Requirements Summary: 1. Experience: 5 years required (3 of which must be in a shift lead/precepting/team lead role in a hospital setting); no first-time travelers — hard stop 2. Cases: Laparoscopic, Robotic, Total Joints, Ortho, Cataracts, Open Heart, Vascular, Neuro/Spine, OB/GYN, Urology, General, General Trauma, Orthopedic Trauma 3. Certifications: BLS, CBSPD or HSPA (IAHCSMM) required; LA fire card within the last 30 days required (not reimbursed) 4. License: CA license must be in hand at time of submission; must disclose any license hits upfront — no open license hits will be approved 5. Education: Not specified 6. EMR: Epic preferred 7. Shifts/Call: Variable shifts; primarily 1900-0330 and 2130-0600; on-call required; weekday on-call 2200-0700; weekend/holiday on-call 0700-1900 and 0700-0700; float required between Main OR and Pavilion (Outpatient Surgery Center); weekends may exceed every other; scheduling requests due to outside employment will not be considered 8. RTO: Must be included at time of submission; maximum 7 days (anything over may delay offer pending client approval); must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day); one pre-approved time off noted 9. Returning Staff: Can return after 3 months; must disclose if previously worked at a Providence facility as permanent employee, traveler, or registry/per diem (include location and dates); can only submit to one Providence facility at a time 10.
