- Unit Notes:***80-mile LOCAL Placements*** LOCAL ADJUSTED BILL RATE: -$8.00 LOCAL ADJUSTED OT BILL RATE: -$8.00 *MUST note in submission if clinician has ever worked for Sanford Health. If clinician previously worked with Sanford, they must have an end date 6 months prior to their submission date.* All clinicians must badge in and out at facility Scrubs: Polo (Ceil Blue, Khaki, Gray, Chocolate Brown) or business casual. General job duties. Types of procedures required. Physical therapy evaluation and treatment; outpatient, inpatient, home care and PTA supervision (NO LTC) Equipment utilized in the unit/dept. EPIC Computer Charting; Ultrasound, Estim,Tractium Ionto Required skills and minimum years of experience. Ortho and Rehab Skills, 1+ years experience Specific licensures and certifications/registrations required. BLS, MN PT License, MN Background Study Shift or schedule requirements. 8-430 with some flexibility; M-F; Minimal weekend/holiday required Floating to other units/departments. Only Therapy, but may float to other locations Bed size/number of rooms for this unit/dept. Nice outpatient department with wellness center; 15 bed hospital Dress Code: Casual Dress; Scrubs Tell us about the unit/department specific training and orientation program. Training for EPIC as needed; manager available at all times Cancellation Policy:Client may cancel up to 12 hours every 13 weeks Charting System:EPIC BSN Required:False License to Submit:Yes Trauma Level:CAH Care Setting:Acute Division:Allied Shift Notes:Tracy is the primary location, but to work between Tracy, Sanford Westbrook, and Murray County Medical Center (in Slayton)8 hour days, M-F with some weekends and holidays in rotation with staff..
