- Key Requirements Summary: 1. Experience: 1 year minimum, recent experience required 2. Cases: Geriatric cases; Home Health only 3. Certifications: BLS required 4. License: License in-hand required with submission 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) experience required; OASIS experience required for staff completing SOC (Start of Care) 7. Shifts/Call: On-call required every 6th weekend; Mon-Fri schedule with weekends optional 8. RTO: Must be submitted at time of application 9. Returning Staff: Former permanent employees must be separated minimum 6 months before eligible to return as traveler; must be approved by HR 10.
