- Key Requirements Summary: 1. Experience: 2 years minimum required for submission 2. Cases: IV trained; experienced with hazardous and non-hazardous preparations; chemo/hazardous drug preparation preferred; sterile and non-sterile compounding required 3. Certifications: CPHt required; IV Certification required; Chemo/Hazardous Drug certification preferred; must be in hand at time of submission 4. License: PA State License required by law; must be in hand at time of submission 5. Education: Not specified 6. EMR: Epic and Pyxis experience required 7.
