Requirements

- Key Requirements Summary: 1. Experience: Minimum 1 year required; acute care experience within the last 3 years is a HARD STOP — do not submit if only recent experience (within last 3 years) is in Retail 2. Cases: Heavy focus on complex patient med rec, order recommendations, and actual changes on behalf of the hospitalist group; "pharmacist as practitioner" model with high degree of autonomy 3. Certifications: Virginia pharmacist licensure in good professional standing required 4. License: VA state license required; B.S. Pharm. or Pharm.D. required 5. Education: B.S. Pharm. or Pharm.D. required; Pharm.D. preferred; Pharmacy Residency from an accredited ASHP institution preferred 6. EMR: Epic experience required; previous experience in a computerized hospital pharmacy setting desirable 7. Shifts/Call: Mon-Fri, ranging anytime between 07:00-23:00 (schedule not finalized); some weekend rotation; weekend and holiday rotation required; on-call not required; no floating within scope; block schedule at manager's discretion 8. RTO: At manager's discretion 9. Returning Staff: Not specified 10. Travel/Local: Locals accepted 11. Background: Not specified 12. Modules: Not specified 13.
