- Key Requirements Summary: 1. Experience: 1 year minimum required 2. Cases: Adult Outpatient, Hand Injury, Soft Tissue Injuries, Tendon Repair, Nerve Injury, Upper Extremity, Arthritis, Lymphedema management, Other Progressive Neurological Disorders; screen for any missing skills or experience 3. Certifications: BLS required (in hand); ACLS required (in hand); NIHSS cert pending accepted (must get during compliance); must be in hand at time noted 4. License: Wisconsin/compact license required; pending license accepted (must obtain during compliance); SSN and DOB required upon submission 5. Education: Not specified 6. EMR: Epic experience preferred 7. Shifts/Call: 35 hours — 10 hours M/W/F (7am-5:30pm) and 5 hours Tuesdays (12:30pm-5:30pm); no call required 8. RTO: Required upon submission 9. Returning Staff: Must be gone 1 year before returning as traveler 10. Travel/Local: 50-mile radius for locals; locals accepted at lower bill rate; please highlight if traveler is local 11.
