- Unit Notes:TCU/Tele + Rehab Float *Looking for someone wish charge experience Will float to 2n, 3n, 4n, and rehab Ratios: TCU 1:8-10 2/3/4 is 1:6 at max Rehab is 1:3-5 WILL REQUIRE WRITTEN CONFIRMATION FROM CLINICIAN IF CLINICIAN IS FULLY COMMITTED AND PREPARED TO ACCEPT WITHIN 24 HOURS - ALL TITER, BKG, DRUG, AND FINGER PRINTS MUST BE DONE WITHIN 3 BUSINESS DAYS OF ACCEPTING OFFER Unit Transitional Care Unit Notes Obtained by # beds Total: 32 Certs EMR Meditech Charge Nurse 1 at times pending on census-1 RN float nurse to assist with admission, D/C, IV s etc. Nurse Aides 2-3 aids 1st and 2nd, 1-2 aids 3rd- per census Weekend Requirement yes Holidays yes Scrub Color navy Tele no Nurse's Read Strips? no Drips no Vents no Hospitalist yes IV team no RT 24/7 no Pharmacy 24/7 yes Shifts 2230-0700 Floating no Floor specific orientation yes Common diagnosis / Types of patients Ortho surgeries (mainly knee/hip) and various-non-acute or critical patients Additional Notes Unit follows LTC regulations, is step down for patient not ready to D/C home or to nursing home after acute care stay or surgical procedure. Patient typical stay 14 days-all patients must have skilled need-various admitting diagnosis Additional Questions Ratios: LPNs: For example today our census is 18-so each nurse has 9, when our census is higher, typically if we have higher end of 20-32 we have 3 nurses on days and evenings, 2 on night RN: The charge each shift is responsible for all patients on unit Since covid, we had capped our census at 21-so we are not cohorting patients currently Step down from acute and post-op care but licensed under LTC regulations.
