- Key Requirements Summary: 1. Experience: 2 years of home health experience with high volume case loads required; no exceptions 2. Cases: 5-6 patients per day; heart failure, diabetes, general medical, high volume lower leg wound care, anticoagulant management, IV therapy; 3 starts of care expected in 12 hours; wound care 6-7 patients; PICC line dressing changes required; wound care required 3. Certifications: BLS required 4. License: Must be active at time of submission; no pending licenses accepted 5. Education: Not specified 6. EMR: Recent OASIS experience required; OASIS exam required prior to interview; must review Oasis E11 (Mobility: G-Section, GG Section, and M18) before exam 7. Shifts/Call: 0800 start required; hourly pay model (not per visit); 8-hour shifts; later start times must be approved by manager before contract start; weekends required every 4 weeks; on-call required; 2 weekend shifts per contract 8.
