Experience: Minimum 1 year of recent, relevant Home Health experience required; OASIS experience required for staff completing Start of Care (SOC); Home Health experience ONLY 2. Cases: Geriatric 3. Certifications: BLS required 4. License: License in hand required at time of submission 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) experience required; OASIS experience required; all charting must be completed by 9am the following morning 7. Shifts/On-Call: Monday-Friday schedule with 1 in 4 weekend rotation for admissions (day off before and after weekend); no nighttime on call; on-call rotation may be required — discuss during interview; minimum 30 points weekly; average 5-7 patients per day (confirm during interview); guaranteed 40 hours 8.
