- Key Requirements Summary: 1. Experience: 1 year required; extensive dialysis experience required 2. Cases: Acute care inpatient dialysis, hemodialysis, peritoneal dialysis/CAPD, apheresis/therapeutic plasma exchange, AV fistula and graft cannulation, tunneled/non-tunneled catheter management; will float to ICU, stepdown, and ED 3. Certifications: ACLS and BLS required; Basic EKG required; copies of all certs must be submitted; must be on hand at time of submission 4. License: Pending license not accepted; map directions from permanent address on license to facility required at time of submission 5. Education: Degree information required — area of study, city and state of institution, and type of degree for all degrees earned 6. EMR: Epic strongly preferred, especially for RNs; willing to consider no Epic experience for difficult-to-fill positions 7. Shifts/Call: 4x10 days; on-call required 1830-0800 varied per unit needs; Sunday on-call 24 hours once a month; every other weekend required; may be placed in charge RN role; will float within scope to St. Charles Bend/Madras 8. RTO: 7-day RTO restriction 9. Returning Staff: Any candidate who cancels with less than 14 days notice will be entered as a system DNR 10. Travel/Local: 75-mile radius rule; locals not accepted; proof of address and map directions from permanent address to facility required at submission 11. Background: 7 years of work history required (8/2015-present), including PTO, medical leave, non-clinical work, and schooling; zero 30+ day employment gaps; all gaps must be accounted for 12. Modules: Non-billable if completed within first 8 hours of orientation; completed during orientation; vary by specialty; required annually 13. Travel Experience: Required; first-time travelers accepted 14. Scrubs: Not specified 15. Flu Vaccine: Required — medical and religious exemptions only; COVID-19 vaccine not required 16. Special Equipment: Fresenius dialysis machine (20008T); conductivity meters; ultrafiltration systems; blood leak detector; air foam detector Critical: Two references from a position held in the last year required; one must be a manager reference from 1/2022-present; pending references not accepted. Submission cannot proceed without all required documentation. Patient ratios: 1:2 days, 1:1 nights.
