Requirements

- Key Requirements Summary: 1. Experience: 2 year minimum, recent experience required; minimum 1 year relevant experience; OASIS experience required for staff completing SOC (Start of Care) 2. Cases: Geriatric; Home Health only 3. Certifications: BLS required 4. License: License in-hand required with submission 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) proficiency required 7. Shifts/Call: On-call rotation may be required - discuss during interview 8. RTO: Must be submitted at time of application 9. Returning Staff: Former permanent employees must be separated minimum 6 months before eligible to return as traveler; must be approved by HR 10.
