Requirements

- Key Requirements Summary: 1. Experience: 1 year required 2. Cases: Extremities, Abdomen/GE, Spine/Pelvis, Chest/Thorax, Musculoskeletal, Lower Extremities, Upper Extremities, Surgery/C-Arm required; Head, Fluoroscopy Studies preferred 3. Certifications: ARRT R, BLS, Fluoro Permit required 4. License: Oregon state license required, can submit with pending 5. Education: Not specified 6. EMR: Epic required 7. Shifts/Call: 3 call shifts per month required; Mon-Thurs 11pm-9:30am shift; block schedules required 8. RTO: 6 days total or 3 consecutive days auto-approved; must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, NYE, New Year's Day); all RTO requests must be made upfront during submittal 9. Returning Staff: 3 months required between permanent and travel; must disclose previous Providence facility employment 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate; must have vehicle for assignment 11. Background: Any background hits must be sent for approval prior to submission 12. Modules: 16 hours average, non-billable; completed during orientation and annually; over 20 hours requires PM approval; self-reported completion recorded in Workramp transcripts 13. Travel Experience: Not specified as required 14. Scrubs: Any color for all units; OR/Cath Lab scrubs provided by hospital 15. Flu Vaccine Requirements: Unknown 16.
