- Unit Notes:Department: Physical Therapist Home Health Certifications Required: BLS, ND Physical Therapy License NO CLINICAL INVOLVEMENT/MANAGER INTERVIEW *Must have current Driver's License & Auto Insurance* Department: Physical Therapy Home Health - # of patients treated per day: 5-7 - Experience or 'Must have' skills: Must have PT related skills & previous home health experience - # Years Experience Required: 1+year - Previous Travel experience Required?: No - Common diagnosis / Types of patients: surgeries, falls, etc. - Home Health and Hospice patients - What areas are they required to cover?: 60mi radius around Valley City, ND - EMR: HomeCare/HomeBase - Is there a PT Manager/Supervisor on each shift?: No - Shift Details: Monday - Friday 8a-5p - Weekend Requirements: No - Holiday Requirements: No - Scrub Color/Dress Code: CSH color scrub or Business Casual - How many shifts of unit/department specific orientation?: 3-5 Radius Rule: 50 miles AND candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity within the past 12 months. RTO needed upfront - Additional RTO will not be approved, including after submissions.
