- Unit Notes:Department: Physical Therapist Home Health Certifications Required: BLS, ND Physical Therapy License NO CLINICAL INVOLVEMENT - MANAGER INTERVIEW ONLY Radius Rule: 50 miles AND candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity within the past 12 months. RTO needed upfront - Additional RTO will not be approved, including after submissions. *Must have current Driver's License & Auto Insurance* Department: PT - Home Health/Hospice How many points a day/week? Do you require travelers to do "start of care", recerts, daily visits? Yes the travel PT may do start of care, recerts and resumption of care. Points are 6.5 pt per day. Do you manage home vents? Are RNs/LPNs expected to have experience managing home vents? n/a "How many years of experience would you require an RN/LPN/CNA to have? Would you accept a first time traveler? Is there any specific type of experience that you are looking for in a candidate for this position? Would prefer someone with Home Health experience and OASIS training. What does someone's typical workload look like? Regular visits 5 -6 per day if day does not involve travel or add on assessments or admissions. If admission-3 other visits. Is there a resource available for them to reach out to while in the field? We have physical therapists in our sister agencies that work the same hours and would be available. Clinical Coordinator and Director available for questions also Do you staff Home Health Aides? No PTA's at this time What are typical shifts and hours? Hours are Monday through Friday 8-5. Holidays and weekends off. How often is someone expected to work the weekend shift? No weekends for PT Are they asked to go into the Holiday Rotation? N/A What are the call requirements for this position? How often? Weeknights? Weekends? N/A Do they cover both Home Health and Hospice? Majority Home Health, may be asked for a safety evaluation or teaching 1 time visit in Hospice situation. Scrubs (color?) Black, Navy blue or grey scrubs. How many shifts of orientation does someone get with a preceptor? Would be working with someone for a short orientation1-2 weeks.
