Requirements

- Key Requirements Summary: 1. Experience: 2 years required for submission; at least 1 year hospital acute care or inpatient acute rehab required; must have experience with neuro patients 2. Cases: CVA, SCI, TBI, Amputation, Cardiac, Oncology, Multi trauma, Elective Spinal SX 3. Certifications: CA PT License, BLS required; all certs must be AHA and in hand at time of submittal; copies required 4. License: CA PT License required; valid state license required at time of submission 5. Education: Not specified 6. EMR: Epic experience required; Epic experience preferred 7. Shifts/Call: Block scheduling; 12 Saturdays per contract 8. RTO: Only one holiday will be approved; two pre-approved time off 9. Returning Staff: Must be gone 1 year to return as traveler 10. Travel/Local: 50-mile travel requirement; locals accepted at lower rate 11. Background: Must disclose any background hits or DUIs prior to submission; 7-year work history verification required 12.
