Requirements

- 1+ years of experience
- Key Requirements Summary: 1. Experience: Minimum 1 year experience required 2. Cases: Seeing all pelvic floor patients, male and female 3. Certifications: BLS required; must be AHA and Red Cross only, RQI cards not accepted 4. License: IL state PT license required; must be in hand at submission 5. Education: Not specified 6. EMR: Cerner experience preferred 7. Shifts/Call: M-F 8:30-5; No on-call required; No weekend requirements 8. RTO: Ideally no more than 5 days; can auto-approve 5 or less days of non-holiday RTO; must work 2 out of 3 major holidays 9. Returning Staff: Previous perm/PRN candidates must be separated for 12 months before applying as traveler 10. Travel/Local: Locals accepted with no radius requirement 11. Background: Background check hits must be noted upon submission for approval prior to offer 12. Modules: Non-billable up to 16 hours; completed pre-start, renewed annually; strict deadlines with 4PM CST Day 1 completion required 13. Travel Experience: Not required 14. Scrubs: Business casual attire or plain solid scrubs 15. Flu Vaccine Requirements: Required - Medical/Religious Exemptions Only 16. Special Equipment: Not specified Critical: Must be in IL 3 business days prior to compliance for fingerprinting.
