Requirements

- Unit Notes:PT - needed 5 8's 4 9's 4 10's - shift 7:30a - 5p 40 GTE Required - EPIC experience - 2 + years outpatient experience - previous travel experience Shift - M-F no weekends 8-10 patients daily 45 minute treatments / 75 minute evals May 18th or June 1st start 13 weeks ( with possibility of extension ) The majority of the patients are orthopedic conditions. The caseload includes mostly adults to geriatrics, however there are a fair amount of sports related rehab. Typical diagnoses include: neck/back pain, ankle sprains, total joint rehab, rotator cuff rehab, ACL rehab, knee arthroscopies, general arthritis conditions, and some stroke rehab. 36 hours/week. Prefer four 9 hour days/week. Must be a licensed Physical Therapist in Ohio, CPR certification. Able to use electronic documentation Cancellation Policy:Able to cancel 1 week's worth of guaranteed hours over a 13 week period Charting System:EPIC BSN Required:False License to Submit:Yes Trauma Level: Care Setting:Outpatient Division:Allied Shift Notes:8's, 9's or 10's 40 GTE M-F only no weekends Required Certifications:BLS Certification
