- Key Requirements Summary: 1. Experience: 1 year required; first-timers accepted 2. Cases: General acute care patients with variety of diagnosis including general surgery, ortho surgery, hip fractures, dementia, deficits in ADLs, falls; primary evaluation and treatment for ADL and UE assessments with focus on functional outcomes 3. Certifications: BLS required 4. License: CA OT license must be in hand 5. Education: Not specified 6. EMR: Epic experience preferred 7. Shifts/Call: 5x8 shifts; facility can call off once per two week pay period; all travelers expected to float between all facilities within given state 8. RTO: Must be included at time of submission; limited to 7 days for both Aya and client offers; must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) 9. Returning Staff: Can return after 3 months; must disclose previous Providence facility employment with locations and dates 10. Travel/Local: 50-mile radius rule applies; locals accepted at lower bill rate 11. Background: Must disclose license/background hits upfront at submission; no open license hits approved 12. Modules: Average 16 hours, non-billable; completed pre-start and annually; over 20 hours requires PM approval; self-reported completion recorded in Workramp transcripts 13. Travel Experience: Not required 14. Scrubs: Navy blue for general staff; L&D/OR provided by facility 15. Flu Vaccine Requirements: Unknown 16. Special Equipment: Adaptive equipment mentioned in skills Critical: LA fire card within last 30 days required. Two references covering 1 year within last 3 years required.
