- Key Requirements Summary: 1. Experience: 1 year required 2. Cases: Covers ER, urgent care referral & IP cases - variety of diagnosis; position also covers general x-ray for part of shift 3. Certifications: ARRT CT, ARRT R, BLS required 4. License: OR license required, can submit with it pending unless specified in job title 5. Education: Not specified 6. EMR: Epic required 7. Shifts/Call: Fri, Sat Sun nights weekly; weekends required Fri-Sun 0830-2100; on-call not required 8. RTO: 6 days total or 3 consecutive days can be auto-approved; must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD) 9. Returning Staff: 3 months between perm and travel; must include response about previous Providence facility employment 10. Travel/Local: 50 mile radius rule; locals accepted at lower rate 11. Background: Any hits must be sent for approval 12. Modules: Non-billable, average 16 hours spent; completed pre-start and annually; over 20 hours requires PM approval 13. Travel Experience: Not specified as required 14. Scrubs: Any color 15. Flu Vaccine Requirements: Unknown 16. Special Equipment: 2 CT units (both GE), contrast injectors x 2, 2 portable XR units (Carestream & GE), 2 C-arms (GE), 2 Phillips general XR rooms Critical: Must have vehicle for assignment.
