- Unit Notes:Treated as a locum - credentialing process will take longer, traveler's compliance packet will need to go to committee before start. Compliant 2 Fridays prior to committee meeting. Committee meets the third Monday every month. "**Local Rate: Candidates living within 80 miles of the facility. Client rates reduced by $8.00/hour.** # of patients treated per day three to six on average General Certification Requirements (BLS, etc.) BLS Other Required certs for all SLP? Do they prefer or require CCC-SLP &/or CCC-A credential? CCC-SLP Common diagnosis / Types of patients Dysphagia, Video Swallow Studies, cognitive decline, CVA, Parkinson's Acute Inpatient, Acute Rehab, Skilled, Outpatient? Acute inpatient, outpatient What areas are they required to cover? Outpatient and inpatient Experience or 'Must have' skills & common job duties 3 years of experience recommended SLP Manager/Supervisor Patty Hautamaki PT Rehab Aides: SLA's no Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) we don't do self-scheduling for outpatient but for inpatient needs. Shift 8-4:30 Monday-Friday Weekend Requirements no Holiday Requirements no Scrub Color/Dress Code Business casual, closed toe shoes, scrubs are allowed no specific color Department specific orientation onsite orientation day one with support from other SLPs in the system as needed *MUST note in submission if clinician has ever worked for Marshfield.
