Requirements
- Required certification: Speech Language Pathologist
- 1+ years of experience
- Unit Notes:**Should be agreeable to cover all 11 counties across both offices (Elizabethtown & Bardstown). It is a very large territory; travel from corner to corner takes over 2.5 hours. MUST HAVE KY LICENSE IN HAND UPON SUBMITTING - LICENSE CANNOT BE PENDING. MUST ALSO HAVE HOME HEALTH EXPERIENCE. Department: SLP # of patients treated per day 6 General Certification Requirements (BLS, etc.) BLS Other Required certs for all SLP? Do they prefer or require CCC-SLP &/or CCC-A credential? CCC - SLP - Licensed in the State working Common diagnosis / Types of patients CVA/Aphasia Acute Inpatient, Acute Rehab, Skilled, Outpatient? N/A What areas are they required to cover? Home Health Experience or 'Must have' skills & common job duties 1 year clinical experience SLP Manager/Supervisor MCS Rehab Aides: SLA's N/A Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) Days (5 days a week 8 hours a day) Weekend Requirements No Holiday Requirements No Scrub Color/Dress Code Professional Department specific orientation 2 weeks with workcoach Additional Notes Additional Questions Facility Follow Up Notes obtained: Phone Implementation, Email, or On-Site?
