- Key Requirements Summary: 1. Experience: 2 years of acute care experience and/or case management in an acute/SAR setting required; experience in specialty must be within the last 12 months 2. Cases: Care coordination, discharge planning, crisis intervention, biopsychosocial assessment, case management; patient population includes Adults and Geriatrics 3. Certifications: LMSW required; LSW and MSW listed; all licenses and certifications must be in hand at time of submission; pending licenses or certificates not accepted; attach all licenses/certificates to submittal email 4. License: LMSW required and in hand at time of submission; Active NJ or Compact License required; Nursys must be within 2 weeks of submittal date; pending license not accepted 5. Education: Not specified 6. EMR: Epic required; charting system experience required 7. Shifts/Call: Every 3rd weekend required; 1 winter holiday and 1 summer holiday required; number of weekend shifts per contract: 3-4; orientation hours: 40; block scheduling not allowed 8.
