- Key Requirements Summary: 1. Experience: 2 years of acute hospital case management experience required 2. Cases: Care coordination and discharge planning required; knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, and POLST required; AIDA and PASRR experience noted; ability to organize and participate in family meetings; must perform both roles of Care Coordination and discharge planning; will be required to cover up to 2 units on weekends 3. Certifications: MSW required 4. License: CA state license preferred; pending license accepted 5. Education: Not specified 6. EMR: Epic experience required 7. Shifts/Call: Every other weekend required; must be available for holiday coverage; must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day); scheduling type listed as Other; float between all facilities within the state required — refusal to float may result in termination or forfeiture of guaranteed hours 8.
