Travel Allied Physical Therapist (PT) Woodland, CA

Epic Travel Staffing is hiring a Travel – Physical Therapist Home Health

Shift: Days, 8:30a-5p, 8×5, 40hrs per week, Mon-Fri
Typically one Saturday per month

Duration: 17 weeks

Requirements:

• Active CA PT License
• Minimum of 2 years of experience as a PT in an acute, home care, or rehab setting
• BLS

Other Details:

• Care setting: Home Health
• Charting system: HomeCare HomeBase (HCHB)
• Provides physical therapy services per a written physician’s plan of care, including assessing/evaluating and updating therapeutic, rehabilitative, and functional status and participating in development of the total plan of care
• Evaluates home environment; assesses muscle strength, mobility, gait, ROM, and rehab potential; directs physical therapy treatment
• Instructs patients and families/caregivers in use and care of therapeutic appliances; may train patients in use of prosthetic devices
• Reports patient reaction to treatment or changes in condition to the physician; prepares and submits clinical and progress summaries
• Supervises homemaker/Home Health Aides participating in the patient’s rehab regime; instructs other personnel and family/caregivers in phases of the PT program
• Identifies patient and family needs for other home health services and refers as necessary
• Participates in peer consultation, case conferences, and ongoing staff competency assessment
• Complies with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other applicable regulations and licensure requirements
• Good oral and written communication skills; strong organizational skills
• Dress code: slacks (khakis acceptable) with polo or button-up shirt if scrubs not worn; any color scrubs acceptable if chosen
