Cases: Geriatrics, Neurology, Orthopedics, Home Health, Post acute care, Post SNF discharge; OASIS experience required (total months and frequency of starts of care for Medicare patients per week must be included with submission) 3. Certifications: BLS (AHA) required; Wound Care and Manual Therapy listed 4. License: HI (Hawaii) License required 5. Education: Not specified 6. EMR: HomeCare HomeBase (HCHB) preferred; productivity requirement of 25 units per week 7.
