- Key Requirements Summary: 1. Experience: Minimum 1 year of relevant experience required; recent experience required; no first-time travelers accepted 2. Cases: Geriatric; average 5-7 patients per day; 6 visits daily or minimum 30 points weekly 3. Certifications: BLS required; Hospice/Home Health Checklist is mandatory 4. License: License in hand required at time of submission; active driver's license required 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) experience required; OASIS experience required for staff completing SOC (Start of Care) 7. Shifts/Call: On-call rotation may be required — must be discussed during interview; no on-call listed as required upfront; Monday orientation 8. RTO: Must be submitted at the time of application 9. Returning Staff: Former permanent employees must be separated for a minimum of 6 months before returning as a traveler; must be approved by HR 10.
