Travel Allied Physical Therapist (PT) Rancho Cordova, CA

Epic Travel Staffing is hiring a Travel – Physical Therapist Home Health

Shift: Days, 8:30a-5p, 8×5, 40hrs per week, Mon-Fri
1 weekend per month requested but not required

Duration: 17 weeks

Requirements:

• Active CA PT License
• 2 years of experience in a healthcare setting
• Home Health experience including OASIS required
• Community/home health experience preferred
• HomeCare HomeBase (HCHB) experience preferred
• BLS
• NPI #

Other Details:

• Care setting: Home Health
• Charting system: HomeCare HomeBase (HCHB)
• Current driver’s license and proof of automobile insurance required
• Treatments performed in patient homes, Assisted Livings, Senior Apartments, and Board and Care homes
• 3 to 5 patients per day; typically 2 Start of Care visits plus 3-4 evaluations or a combination totaling 3-5 (Start of Care weighted at ~3.75-4 hours covering travel, documentation, and case management)
• Diagnoses treated: Ortho, Cardio, Pulmonary, Oncology, Neuro, Geriatric, and Amputations; no pediatrics or Hospice
• Will supervise up to 2 PTA/COTAs
• Coverage area depends on unit needs; teams service the entire territory including Woodland/Yolo County, and clinicians may cover patients on a different team as needs arise
• Must be able to work in home environments that may be cramped, hot/cold, or cluttered
• Good oral and written communication skills; strong organizational skills
• Managed by an RN Manager with support from the Regional Rehab Leader/Educator for therapy-related questions
• Dress code: business professional; closed-toe shoes required; no jeans, tank tops, or logos
