- Unit Notes:Profiles go directly to the unit manager for consideration Need 2 sup refs (charge accepted) at submittal; 1 w/in 12 mo. & 1 w/in 3 years worked Copy of the DL is needed at time of submittal to validate legal name - Pulse must reflect the name on a gov't issued ID - no middle initial or middle name Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered. EMR HomeCare Home Base (HCHB) experience preferred. Home Health experience including OASIS required. 2 years experience in a healthcare setting required -- Community/home health experience is preferred. Required certs: BLS & NPI # **Also needs a CA PT license** HH requires a current driver's license & proof of automobile insurance. Good oral and written communication skills; has good organizational skills. **Mileage is billable per GSA guidelines. Personal timesheets showing mileage must be signed by home health supervisors. There are meetings twice a month to have hard copies signed (if they do not see supervisors on a regular basis). They may also send photocopies of timesheets with mileage listed to the supervisors via email for approval. Timesheets need to be sent to [email protected] in order to bill. **Accrued mileage begins after the clinician departs from their first assignment. Travel between the clinician s housing and the first HH visit does not qualify for reimbursement. Home Health PT: all treatments in homes, Assisted Livings, Senior Apartments, Board and Care homes. - 3 to 5 patients per day depending on the visit. 2 Start of Care visits per day, of 3-4 evaluations or any combinations to make 3 to 5. We do not have a point system but visits are weighted with a time that covers travel, documentation and case management. Example a Start of Care is weighted at 3.75 to 4 hours total. - BLS is required w/ no other certs required besides valid PT License. - Diagnosis: varied no pediatrics or Hospice. We treat Ortho, Cardio, Pulmonary, Oncology, Neuro, Geriatric, Amputations. - Area Depends on needs of the Unit. We have 3 teams that service the entire service territory. We see patients from all of Mercy area hospitals and our Mercy Community doctors, primarily serving Mercy San Juan Hospital in Sacramento. - Territories are grouped to decrease excessive mileage and drive time to increase efficiency and case continuity for our patients. Despite being placed on a Team in the Unit, we go where the patients are and sometimes cover patients on a different team as the need arises. Woodland has a territory that service the Yolo Co. Woodland has 2 service teams. - PT is Managed by a RN Manager with support from the Regional Rehab Leader/Educator for Therapy related questions. - There are no "Must Have" skills required but need to be able to work in home environments that are sometimes cramped, hot/cold or infested with clutter/bugs. - PT will supervise up to 2 different PTA/COTAs - Shift is 0830-5 and work is expected to be completed at the end of this shift. - There is 1 weekend/month requested but not required for PT. - Holiday work is required but we could be flexible if time off is requested. - Scrubs not provided but recommended. Dress code is business professional. Must wear closed toe shoes, no jeans, not tank tops or shirts with logos. - There will be a robust 3 week orientation process with online learning and precepting in the field. Solo work is expected the end 2nd week if ready of by the end of 3rd week for sure.
