- Key Requirements Summary: 1. Experience: Minimum 1 year required; first-time travelers accepted 2. Cases: Pelvic floor patients, male and female; outpatient setting 3. Certifications: BLS required; must be AHA or American Red Cross not specified, but BLS is required 4. License: Active IL state PT license required; must be in hand or already applied at time of submission 5. Education: Not specified 6. EMR: Cerner experience preferred 7. Shifts/Call: M-F 8:30-5; no on-call required; no weekend requirement; no floating within facility 8. RTO: Ideally no more than 5 days 9. Returning Staff: Not specified 10. Travel/Local: Locals accepted; no radius restriction; active IL license required 11. Background: Must be in IL 3 business days prior to compliance HDD for fingerprinting requirements 12. Modules: Not specified 13. Travel Experience: Not required; first-timers accepted 14. Scrubs: Business casual attire or plain solid scrubs 15. Flu Vaccine Requirements: Required; Medical/Religious Exemptions Only 16.
