- Key Requirements Summary: 1. Experience: Minimum 1 year experience required (18 months exp. required per general facility policy) 2. Cases: Pelvic floor patients, male and females; outpatient clinic 3. Certifications: BLS and IL state PT license required; must be AHA or Red Cross only, RQI cards not accepted 4. License: Must have IL license in hand at submission; must be in IL 3 business days prior to compliance for fingerprinting requirements 5. Education: Not specified 6. EMR: Cerner experience preferred 7. Shifts/Call: M-F 8:30-5, no weekends, no on-call required, no floating within scope 8. RTO: Auto-approve 5 days of non-holiday RTO or less, anything more requires facility approval; candidate must work 2 out of 3 major holidays 9. Returning Staff: Any permanent or PRN employee of Memorial must be away for 12 months before applying as traveler 10. Travel/Local: Locals accepted with no radius restrictions 11. Background: Background policy to be noted prior to submission to seek approval prior to offer 12. Modules: Non-billable up to 16 hours, after 16 hours billable; completed pre-start then annually; sent Friday before start, must be completed by 4pm CST on first day; strict deadlines enforced 13.
