- Key Requirements Summary: 1. Experience: Minimum 1 year experience required 2. Cases: All pelvic floor patients, male and females 3. Certifications: BLS and IL state PT license required; must be AHA and Red Cross only - RQI cards not accepted 4. License: IL license required; MUST have IL license in hand at submission 5. Education: Not specified 6. EMR: Cerner experience preferred 7. Shifts/Call: M-F 8:30-5; no on-call required; no weekend requirements 8. RTO: Ideally no more than 5 days; can auto-approve 5 or less days of non-holiday RTO; anything more requires facility approval; candidate MUST WORK 2 out of 3 major holidays 9. Returning Staff: Previous Perm/PRN candidates must be separated for 12 months before applying as traveler 10. Travel/Local: Locals accepted with no radius restriction 11. Background: Background check hits/review must be noted upon submission for approval prior to offer 12. Modules: Non-billable up to 16 hours; completed pre-start, renewed annually; strict deadlines with candidates pushed if not completed on time (4PM CST on Day 1); first day is virtual at-home day 13.
