Job Description Performs initial and ongoing assessments of patient's condition. Implements the treatment plan using appropriate modalities. Documents subjective, objective, and assessment findings and plans patient progress. Educates and counsels the patient and family/caregiver. Plans and/or assists with patient discharge from therapy services and treatment setting. Provides information regarding appropriate selection/use of adaptive equipment and support programs. Supervises and delegates the care provided by physical therapy assistants according to established licensing laws, state regulations and practice standards. Some employees in this job may operate a motor vehicle to conduct MaineGeneral Health business. Maine State Physical Therapy licensure Masters or Doctorate Degree in Physical Therapy from an Accredited Physical Therapy Program required. *Physical Therapy Graduates with a Doctoral Degree from an Accredited Physical Therapy Program are required to: Provide Authorization to Test (ATT) letter from the Federation of State Boards of Physical Therapy upon hire and, Obtain a Maine State Physical Therapy licensure within 60 days of test date and, Obtain a Supervisory Agreement Letter signed by the supervisory MaineGeneral Physical Therapist until licensure is attained (to be maintained by department) Maine Adult Protective Services Background Check required for ALL employees at Alzheimer's Care Center, Gray Birch, & The Inn at City Hall. Cancellation Policy:Up to 2 shifts for assignments less than 13 weeks and up to 3 shifts for assignments 13 weeks or longer.
