- Key Requirements Summary: 1. Experience: 2 years required for submission; PGY1 Pharmacy Residency Program completion required; new grad ok 2. Cases: Not specified 3. Certifications: All certs must be AHA and in hand at time of submittal; copies required 4. License: PHARMD-Licensed Pharmacist OR PHARMR-Registered Pharmacist; valid state license required at time of submission 5. Education: Doctorate: Graduation from an accredited school of Pharmacy; equivalent experience accepted in lieu of required degree 6. EMR: Epic experience preferred 7.
