Cases: Outpatient infusion (non-hazardous and hazardous including oncology); order verification, IV infusion compounding, oncology and non-oncology treatment plan evaluation, insurance coverage verification, patient education, adverse reaction management 3. Certifications: Chemo/Hazardous Drug certification required; Oregon Pharmacist License required 4. License: Oregon Pharmacist License required 5. Education: Not specified 6.
