- Key Requirements Summary: 1. Experience: 1 year required; specifically at least 1 year of experience with outpatient infusion 2. Cases: Outpatient infusion required; includes order verification of non-hazardous and hazardous (oncology) infusion medications, IV infusion compounding product evaluation and verification, evaluation of oncology and non-oncology treatment plans, insurance coverage evaluation, patient education, and adverse reaction management 3. Certifications: Chemo/Hazardous Drug certification required; Oregon Pharmacist License required 4. License: Oregon (OR) Pharmacist License required; can submit with it pending 5. Education: Not specified 6. EMR: Epic required 7. Shifts/Call: Block schedule required; no on-call required; no weekends required; ratios 1:10 days, 1:10 weekends; 0 NBO hours; 48-hour shifts not approved 8. RTO: 6 days total or 3 consecutive days auto-approved; anything beyond 6 days may delay offer pending client approval; must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, NYE, NYD); all RTO must be requested upfront at submission; post-submittal RTO may not be granted 9.
