Requirements

- Key Requirements Summary: 1. Experience: 1 year required 2. Cases: Inpatient pharmacy experience preferred 3. Certifications: Chemo/Hazardous Drug, Oregon Pharmacist License required 4. License: Oregon Pharmacist License required; can submit with pending license 5. Education: Not specified 6. EMR: Epic experience required 7. Shifts/Call: On-call required for overnight hours; weekends required 0730-1700; block scheduling required 8. RTO: 6 days total or 3 consecutive days auto-approved; must commit to working 3 of 5 holidays; must request all RTO upfront during submittal 9. Returning Staff: 3 months required between permanent and travel; must disclose previous Providence facility employment 10. Travel/Local: 50-mile radius rule; locals accepted at lower rate; must have vehicle for assignment 11. Background: Any background hits must be sent for approval prior to submission 12. Modules: Average 16 hours, completed pre-start and annually; non-billable; over 20 hours requires approval; self-reported completion recorded in Workramp transcripts 13.
