- Key Requirements Summary: 1. Experience: 1 year required; home health experience required 2. Cases: Med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA, BKA, ADLs; Skilled Home Care (Medicare, Medicare Advantage, and Commercial insurance) 3. Certifications: BLS required; OASIS training preferred; LVST noted; all copies required at submission 4. License: Valid driver's license required 5. Education: Not specified 6. EMR: Charting system experience required; BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system used 7. Shifts/Call: 6-8 home visits per day (minimum 6 points per day); 1-2 weekends per contract; self-scheduled based on HomeCare office needs 8. RTO: Not specified 9. Returning Staff: Not specified 10.
