Requirements

- 1+ years of experience
- Unit Notes:Profiles are sent directly to the manager need 2 sup refs (charge accepted) at submittal; 1 w/in 12 mo. & 1 w/in 3 years worked Copy of the DL is needed at time of submittal to validate legal name - Pulse must reflect the name on a gov't issued ID - no middle initial or middle name Home Health OT 2 years experience in a healthcare setting required Required certs: BLS & NPI # **Also needs a CA OT license** HH requires a current driver's license & prof of automobile insurance. *****We would consider a COTA with home health experience if there are no OT applicants. ***** four visits per day on average; combination of Evals and Revisits Coverage area includes north-eastern Sac into Folsom AND Woodland - Yolo county service areaqualify for reimbursement. Cancellation Policy:Hospital can cancel up to 4 shifts per 13-week assignment, 3 shifts for assignments that are less than 13 weeks Charting System:Cerner BSN Required:False License to Submit:Yes Trauma Level: Care Setting:Home Health Division:Allied Shift Notes:M-F 0830-1700, First to Float Required Certifications:BLS Certification
