- Unit Notes:OSF ITEMS OF NOTE: - Local Rate: Applies to clinicians with permanent addresses within 50 miles (-$25 to regular and OT bill rates) - OSF Call Off Policy: Equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift - Non-billable Orientation Hours: 12 hours, guaranteed hours will not apply during orientation REQUIREMENTS FOR SUBMISSION: - Last four (4) digits of clinician's SSN - Clinician's middle initial - Requested time off - Active IL license or confirmation that application process has been initiated - Your candidate understands OSF is a Drug-Free Campus and they must produce a negative drug screen for all panels - # of patients treated per day: 45 treatment times and 90 minute evaluations. Up to 10 patients per day. - General Certification Requirement: BLS (can be completed on hire as well). License for state of Illinois for Occupational Therapy. - Other Certs Required for all OT: N/A - Common diagnosis / Types of patients: Individuals with sensory processing challenges, brachial plexus injury, fine motor delay, Autism, ect. - Acute Inpatient, Acute Rehab, Skilled, Outpatient?: Outpatient - What areas are they required to cover?: Outpatient OT for birth - 18 - Experience or 'Must have' skills & common job duties: Pediatric experience for at least 1 year - Is there an OT Manager/Supervisor on each shift?: There will be access to some, but they may not always be onsite. - Do you staff Rehab Aides: COTAs?: No - Shifts & Scheduling: Monday-Friday (3 days 8-430 and 2 days 9-530) - Weekend Requirements: If interested but not expected. Must have IP experience if willing to offer weekends - Holiday Requirements: No - Scrub Color/Dress Code: Scrubs are okay and preferred. - Department specific orientation: Yes - have binder and will have preceptor.
