- Key Requirements Summary: 1. Experience: Minimum 3 years required; first-timers not accepted. Level I trauma experience required. General OR and trauma experience required. 2. Cases: Bariatric (laparoscopic assisted); General Surgery (appendectomy, cholecystectomy, colon resection, hernia repair, laparoscopic assisted, mastectomy, thyroidectomy, Whipple); Neurology (cervical fusion, craniotomy, laminectomy/discectomy, lumbar fusion); Orthopedic (arthroscopy, carpal tunnel release, closed/open reduction of fractures); Plastic Surgery (free flap - microvascular); Transplant (organ donation); Urology (cystoscopy, nephrectomy, prostatectomy); Trauma (amputations, gunshot/stab wounds, MVA). All service lines except CV. HARDSTOP - do not submit if candidate is missing any required skills. 3. Certifications: BLS required; CNOR preferred. All certifications must be in hand. 4. License: Active IL license required - must be in hand or in application process at time of submission. 5. Education: Not specified. 6. EMR: Cerner preferred. 7. Shifts/Call: 4x10s, 0645-1715. On-call required - 1 per week (though less frequent now). Weekend requirement - 1 per month. No block scheduling. Will float within skill set (rarely). Will not work with CV service lines. 8. RTO: All requested time off must be reviewed and approved by manager. 9. Returning Staff: Not specified. 10. Travel/Local: Locals not accepted; must live at least 50 miles from facility to apply. 11. Background: Not specified. 12. Modules: Orientation is 40 hours. Parking permit required at no cost; form completed during orientation. 13. Travel Experience: Required. 14. Scrubs: Provided by the facility. 15. Flu Vaccine Requirements: Required - Medical/Religious Exemptions Only. 16. Special Equipment: Electrosurgical units, fiber optic light sources/cords, Hana tables. Critical: HARDSTOP - Do not pass or screen candidates missing any required skills. Guaranteed hours clause: client may call off up to 36 hours per 13-week assignment without pay; excess cancellations are billable. On-call in lieu of call-off counts as a called-off shift.
