- Key Requirements Summary: 1. Experience: 2+ years in OR setting required 2. Cases: ENT, GYN, Ortho, General surgeries; Pediatrics 6 years and older; HARDSTOP for Ophthalmology - must have 3/4 skills for cataracts, retina, cornea, oculoplasty 3. Certifications: BLS required, CNOR required 4. License: Not specified 5. Education: Not specified 6. EMR: Epic experience required - HARDSTOP 7. Shifts/Call: 4x10, 0645-1715 M-F, variable days, no holidays; No on-call required 8. RTO: Minimal, cannot accommodate RTO during first 2 weeks 9. Returning Staff: Prior UW staff requires 18 months separation 10. Travel/Local: Locals accepted at same bill rate; Candidates float within hospital 11. Background: Must disclose any hits when submitted 12. Modules: Non-billable, completed pre-start; RN 5-6 hours, Allied 2-3 hours; Additional 6 hours for WA suicide prevention if working with MSL 13. Travel Experience: Yes, travel experience required 14. Scrubs: Any solid color scrubs; OR provides light blue scrubs 15. Flu Vaccine Requirements: Unknown 16. Special Equipment: Alcon unity for eyes; HARDSTOP - needs 3/4 skills checklist for eyes Critical: EPIC experience is a HARDSTOP requirement. Ophthalmology experience with 3/4 skills for eyes is HARDSTOP - redirect if not met. Two references required at submittal (prefer supervisory). Compliance deadline 3 business days prior to start. Inpatient or ambulatory work history sufficient, ambulatory recent preferred.
