Cases: Case management of 20-25 patients with an expectation of 6 visits per day; may be used as revisitor if agency needs change during contract. Patient types include adult med surg, oncology, cardiac, neurological, respiratory, wound care, GI/GU 3. Certifications: BLS required 4. License: Not specified 5. Education: Not specified 6. EMR: Epic required; Oasis experience also required 7. Shifts/Call: 0800-1700; 40 hours week
