- Key Requirements Summary: 1. Experience: Minimum 12 months of Home Health experience within the past 3 years; minimum 1 year of relevant experience overall; no first-time travelers 2. Cases: Home Health ONLY; OASIS experience required for staff completing Start of Care (SOC) 3. Certifications: Not specified; Hospice/Home Health Checklist is mandatory 4.
